Learning Objectives

1. Identify clinical indicators and disease trajectories that signal an appropriate time to refer patients to palliative medicine, including both early integration and late stage referral scenarios.

2. Describe the scope, services, and interdisciplinary roles of the palliative care team and explain to patients and families what they can expect when engaging with palliative care.

3. Demonstrate effective, compassionate communication strategies for initiating palliative care conversations with patients and families, including language that supports shared decision-making and aligns care with patient values and goals.
